My name is Jan Seeley. I am an August, 1994 graduate of The Brian Utting School of Massage, Cortiva Institute in Seattle , Washington. My studies included Massage Theory and Practice, Anatomy and Physiology, Kinesiology, Pathology and Contraindications, Clinical Practice , and advanced techniques, including pregnancy massage, reflexology, sports massage, and manual lymphatic drainage. I have had a private practice in West Seattle, Washington since October, 1994.

 I am currently a provider for the following insurances:  Aetna, Cigna, First Choice Health Network,  Premera Blue Cross, Lifewise of Washington, Group Health Cooperative through Healthways, Inc., Alliant Plus, Washington State Labor and Industries(Workmen's Comp), and Personal Injury (PIP/MVA) claims. Insurance claims require a physician's referral or prescription. Please check with your insurance Customer Service to review your massage benefits before making an appointment.

 

If you wish to use insurance to pay for your massage, please have a prescription stating the frequency and duration of your treatment, as well as your diagnosis and diagnosis code.

 

Disclaimer: A missed/no show appointment without a phone call will result in the client being billed for the entire session. Insurance will not apply to that billing. So please, if you cannot make it, please let me know as soon as possible.
